Try-Out Schedule for Lewis County Middle School

Any student planning to participate in a school sport must have a current athletic physical on file prior to participating in try-outs, practices, or games. This requirement is set by the Tennessee Secondary School Athletic Association (TSSAA) to ensure the health and safety of all student-athletes. Physicals must be up to date and submitted before any athletic participation is allowed.

The athletic physical below is the document that the physician will need to complete.

Instructions for completing athlete registration (the parent and student portion) are provided in the second document.
